How does the NTI actually work?

The NTI is a small, clear, custom-fitted, plastic device that is about 1 inch wide. It is placed over the upper or lower front teeth (see picture at the top of page). The opposing front teeth contact against this device in a way that keeps the back teeth from contacting each other. If those back teeth are not allowed to bang, or clench together, the powerful muscles in your temples can't tighten to produce discomfort or pain. It is this clenching that is considered a significant trigger for even migraine headaches! Studies show that the front teeth are the main ones that give feedback to allow these temple and cheek muscles to RELAX, and this is why the NTI contacts only the front teeth.

Frequently Asked Questions About the NTI Night Guard

TMJ jaw pain is localized in the mandible joint Q: What does "NTI" stand for?
A. Nociceptive (relating to the perception of things that could be harmful); Trigeminal (the nerve that controls the major muscle of mastication, or chewing); Inhibition. In other words, NTI = Noiciceptive Trigeminal Inhibitor, a device that inhibits the electrical signals to the large cheek muscles, and thereby comfortably controls nightly teeth grinding, bruxing, clenching, and the subsequent pain associated with it.

Q. Won't teeth supra-erupt while wearing the NTI at night?
A. No. The NTI device is worn primarily while sleeping. Teeth can not supra-erupt since the teeth are stimulated by eating (the night-guard is NOT worn while eating)

Q. I heard that some people may get a little open bite. Is this true?
A. Although uncommon, if there is any occurrence of a small open bite (space between the upper and lower front teeth) during treatment, this is due NOT to super-eruption of the back teeth, but to a better and more healthy repositioning of the TMJ jaw joint. It also only occurs in the reduction of TMJ symptoms. These tiny openings are hardly noticeable! Note: Dr. Reifman has fabricated over 400 NTI night guards, and has seen only a handful of open bites. In those cases, the patients are the happiest they've been, due to suppression of their clenching, headaches, and jaw pain when wearing the nightguard.

Q. Can you swallow it?
A. Made properly, the NTI device cannot be removed without using your hands. In fact, during the delivery of the NTI, Dr. Reifman will tell the patient to "try as hard as you possibly can to push out the guard with your tongue, lips and teeth! Take 10 seconds, a minute, or however long you wish to try to push it out. The pressure and effort you are trying to loosen the night guard is FAR more than any possible clenching you would ever do in your sleep." Patients simply can't push out or loosen the guard... it is secure. Yet, the removal of it in the morning is easily done using only your hands.

Q. Is there a difference between the NTI and the traditional dental night guard device or TMJ splint?
A. Yes. Traditional flat-plane splints and night guard devices provide a surface for you to "bite down on." New studies show, however, that these devices can actually increase the intensity of your clenching reflex, often to over 104% of your normal bite.* The NTI is smaller and more unobtrusive than a night guard and appears to be more successful at doing what night guards try to do. This is because all of your opposing teeth contact against a night guard, so you can still clench and grind and tire out your temporal muscles. Your back teeth cannot contact when you wear an NTI, so the temporal muscles cannot contract.

With the NTI in place, your teeth are protected from excessive force, and the temporal muscles get a chance to relax. In fact, high-intensity clenching has recently been found to be the real cause of the problem. With the advent of the revolutionary NTI device, we can begin to solve the real problem: actually preventing your back teeth from clenching intensely and coming into contact with each other. Stop intense clenching, stop the source of the pain. NTI suppresses the clenching reflex by preventing the posterior teeth from contacting each other. There is no surface to bite down on or bite into. The temporalis muscles are kept from contracting with their usual intensity. The clenching reflex is suppressed and almost eliminated. The NTI helps to keep the clenching and bruxing from happening!

Whiplash and TMJ Injuries

Whiplash may produce delayed jaw pain!

TMJ can cause severe migraine headaches CHICAGO — One in three people exposed to whiplash trauma is at risk of developing delayed TMJ symptoms that may require treatment, according to research recently published in The Journal of the American Dental Association.

Have you ever been in an automobile accident, or know a friend who has hurt their spine through whiplash? Cervical spine injury, caused by so-called whiplash, has had an alarming increase in recent years, no doubt due to increased traffic density, and the accompanying frequency of auto accidents. Whiplash injuries are capable of producing a large number of symptoms. These symptoms can include headache, neck pain, shoulder pain, dizziness and tinnitus. Jaw pain may or may not be a prevalent complaint. However, doctors seeing whiplash patients must be aware that whiplash can cause TMJ injury and that the symptoms can mimic cervical (neck bones) injury.
